Confidentiality Statement:

I understand that by my electronic signature above, I am obligated to the following conditions:

- I will protect the confidentiality of all clients in this facility;

- I am not permitted to disclose any information obtained during my visit regarding clients of Catholic Charities Diocese of Kalamazoo (CCDOK). Federal law prohibits this disclosure. Criminal penalties will apply in cases where information is disclosed without consent.

- I agree to remain only in the areas approved for my visit, and if supervision of my visit is required, I agree to cooperate with such.

- Unless specifically approved by the Security Officer/CCDOK staff, I agree not to transport any data, records, equipment, or other media off of the agency premises.

 

Media/Photo Release

I grant permission to Catholic Charities Diocese of Kalamazoo to use my image (photographs and/or video) for use in media publications which may include, but is not limited to, the following: Videos, Email Blasts, Recruiting Brochures, Newsletters,  General Publications, Social Media (i.e. Website, Facebook).

 

I hereby waive any right to inspect or approve the finished photographs or electronic matter that may be used in conjunction with them now or in the future, whether that use is known to me or unknown, and I waive any right to royalties or other compensation arising from or related to the use of the image.

 

I have read this release and I fully understand the contents, meaning and impact of this release. I understand that I am free to address any specific questions regarding this release by submitting those questions in writing prior to signing, and I agree that my failure to do so will be interpreted as a free and knowledgeable acceptance of the terms of this release.

 

This consent remains in effect indefinitely unless notice of revocation of authorization by consenter is submitted in writing to Catholic Charities Diocese of Kalamazoo.  

 

I certify that all information provided by me is correct, accurate and complete to the best of my knowledge. I authorize both CCDOK and persons listed (references, schools, current (unless noted) and former employers and any others with whom you desire to check) to communicate with regard to any relevant information that may be required to reach a decision on my volunteering with the Agency.
