Volunteer Agreement and Consent Form
I understand that I am not considered an employee of SafeRide America, Inc., while performing volunteer work for the organization. I further understand that as a volunteer, I am not covered by Workers’ Compensation insurance (medical coverage or loss of wages) for injury that may occur while I am acting as a volunteer.
In connection with my voluntary involvement in activities undertaken for, and with the participated and support of SafeRide America, Inc., a nonprofit organization, I hereby agree, for myself, my heirs, assigns, executors, and administrators to release and discharge SafeRide America, Inc., its officers and directors, employees, agents, and volunteers from all claims, demands, and actions for injuries sustained to my person and/or property as a result of my involvement in such activities, whether or not resulting from negligence and I agree to release and hold SafeRide America, its officers and directors, employees, agents, and volunteers harmless from any cause of action, claim, or suit arising there from.
I hereby attest that my attendance and involvement in such activities is voluntary, that I am participating at my own risk, and that I have read the foregoing terms and conditions of this release.
I hereby confirm, represent, and warrant that I have never been convicted of or charged with a violent crime, child abuse or neglect, child pornography, child abduction, kidnapping, rape or any sexual offense, nor have I ever been ordered by a court to receive psychiatric or psychological treatment in connection therewith.
I hereby confirm, represent, and warrant that I have not consumed or ingested in the past 24 hours any alcohol, illegal drugs or narcotics of any kind and I am not under the influence of any prescription drugs that impair judgment or my ability to safely operate a vehicle.
I hereby authorize SafeRide America, Inc. to receive any criminal history record information pertaining to me, which may be in the files of any state or local criminal justice agency in Georgia.
I understand that I am responsible for any loss of damages related to the car topper or the GPS/phone unity that I may be assigned. I understand that I am expected to have a working DC power connector (12 V receptacle).
I understand the dress code for volunteers at SafeRide America is business casual, with jeans being acceptable while hats are not. I also understand I am expected to wear a collared shirt and that shorts are only acceptable if serving at an event.
I am aware that I am expected to communicate with the assigned SafeRide America department in the event that I am unable to meet my obligated hours and that failure to do so may result in being removed from any and all schedules.
