I understands that the scope of my relationship with Our Friends Place is limited to a volunteer position and that no compensation is expected in return for services provided by that Our Friends Place  will not provide any benefits traditionally associated with employment; and that I am responsible for my own insurance coverage in the event of personal injury or illness as a result of my services to Our Friends Place.  

 

1. Waiver and Release: I, the Volunteer, release and forever discharge and hold harmless Our Friends Place and its successors and assigns from any and all liability, claims, and demands of whatever kind of nature, either in law or in equity, which arise or may hereafter arise from the services I provide to Our Friends Place. I understand and acknowledge that this Release discharges Our Friends Place  from any liability or claim that I may have against Our Friends Place with respect to bodily injury, personal injury, illness, death, or property damage that may result from the services I provide to Our Friends Place  or occurring while I am providing volunteer services.  

 

2. Insurance: Further I understand that Our Friends Place  does not assume any responsibility for or obligation to provide me with financial or other assistance, including but not limited to medical, health, or disability benefits or insurance. I expressly waive any such claim for compensation or liability on the part of Our Friends Place  in the event of injury or medical expenses incurred by me.  

 

3. Medical Treatment: I hereby release and forever discharge Our Friends Place  from any claim whatsoever which arises or may hereafter arise on account of any first-aid treatment or other medical services rendered in connection with an emergency during my tenure as a volunteer with Our Friends Place.  

 

4. Assumption of Risk: I understand that the services I provide to Our Friends Place  may include activities that may be hazardous to me including inherently dangerous activities. As a volunteer, I hereby expressly assume risk of injury or harm from these activities and release Our Friends Place  from all liability.  Our Friends Place  is not responsible for personal items in or around the building, inclusive of any Our Friends Place  designated parking areas. Each volunteer must safeguard all belongings.  

 

5. Media Release: I give permission to Our Friends Place to record and capture audio, video, photography, or any other means (including digital media), whether now known or hereafter developed (“Recordings”).  I agree that Our Friends Place is and will be the sole copyright owner of any such Recordings produced by Our Friends Place, and I hereby irrevocably assign and will assign any right, title, and interest I any have in and to such Recordings to Our Friends Place. I agree that Our Friends Place may use my name, image, likeness, voice, testimonials, and any other biographical information provided or publicly available in connection with Our Friends Place, including as incorporated in or associated with the Recordings. I agree, on behalf of myself and my agents, guardians, representatives, heirs, assigns and next of kin (collectively, “Releasing Parties”) to waive, release, discharge and hold Our Friends Place harmless from and against any and all losses, liabilities, claims, costs, and damages suffered by Our Friends Place that may arise from or in connection with the permissions granted in this release, including but not limited to claims based on defamation, false light, invasion of privacy, right of publicity, tort or breach of contract. I waive any and all rights or claims to compensation from Our Friends Place for the permissions granted in this Release. 

 

6. Confidentiality: I understand it is my responsibility to keep all Our Friends Placeresidences confidential for client safety. I will not give out the addresses without prior authorization from Our Friends Place  staff. I agree to refrain from communicating information about clients to anyone other than Our Friends Place  staff. If there is a clear and immediate danger to an individual or to society, I will consult the Executive Director or designee prior to communicating information about a client, and then only to the concerned individual and appropriate family members, professional workers, or public authority. If authorized to store and/or dispose of written client information and /or records according to established practices in the agency, I agree to protect the safety and confidentiality of all clients.  

 

As a volunteer, I expressly agree that this release is intended to be as broad and inclusive as permitted by the laws of the State of Texas and that this release shall be governed by and interpreted in accordance with the laws of the State of Texas. I agree that in the event that any clause or provision of this release is deemed invalid, the enforceability of the remaining provisions of this release shall not be affected.  

• All volunteer applications are used for Our Friends Place purposes only.  
• Volunteers are prohibited from being under the influence of any illegal substances in or around the administrative office building, and transitional living centers, and any site where Our Friends Place serves clients. Further, if volunteers suspect other volunteers or anyone affiliated with Our Friends Place to be under the influence of any illegal substances, then the volunteer must report such to the Our Friends Place  staff immediately.  
• Volunteers understand and acknowledge their responsibility to immediately report to an Our Friends Place staff member any suspected incident of abuse of a client.  
• Abuse: Non-accidental infliction or threat of infliction of physical, emotionalor mental harm to a client living in one of Our Friends Place’s transitional living centers or a client participating in non-residential programming..  
• Under no circumstances willOur Friends Place  tolerate any form of sexual harassment or misconduct. 
• Volunteers must be aware of where they are and what they are doing at all times. If any kind of unwarranted incident arises, please contact an Our Friends Place staff person   

 

I verify that I have the required authority to enter into this release and to grant the permissions stated above, including any necessary permissions from my employer or other affiliated organization.  

I hereby consent to the terms of participation as described above.
