I know that the Bible is the inerrant, infallible, eternal Word of the One True Living GOD of Abraham, Isaac, and Jacob. I know that Jesus Christ is the only Son of God, that He was born of a virgin, died on the cross for the remission of sins, and rose again on the third day according to the scriptures.  He will be coming back in glory to judge the living and the dead. I know that there is a real Heaven and a real Hell (John 3:16-21). There are not many paths to salvation - there is only One.  As Jesus said, “I am the Way, the Truth, and the Life. No one comes to the Father except through me.”  I am pro-life from conception to natural death.  I know that marriage as GOD ordained it is between a man and a woman only (Matthew 19:4-6).

 

I agree to:

 

1.  Meet the Team Leader or Assistant at the event area at the time instructed.

2.  Be on time and serve on the shift I am assigned to.

3.  Follow the "List of Things Volunteers Should Know".

4.  Not leave the area I am assigned to without being dismissed by the Team Leader or Assistant for that area.

5.  Not take instruction from anyone other than the PHM Event Staff, Volunteer Coordinator, Volunteer Coordinator Assistants, Volunteer Team Leader, or in the case of an emergency only, the security personnel.

6.  Wear the appropriate wristband, making sure it is visible, during the entire time I am at the festival.

7.  Not bring any pets to the PHM Event.

8.  Not have any children or friends with me during the time I am volunteering.

9.  Not enter any giveaways (and communicate to my family members - mom, dad, children, brother, sister, or legal guardian - they cannot enter any giveaways) during the NightVision Festival.  If a volunteer or his/her family member wins any item, that item must be returned and a new winner will be drawn.

10. Voluntarily release, and forever discharge, Praise Him Ministries, Inc. and its directors, officers, employees and agents, and any individuals associated with or working for Praise Him Ministries relating to NightVision (hereafter collectively referred to as "PHM"), from any and all liability claims, actions or rights of action which are in any way related to or arise out of NightVision or my participation in any festival activity.  I recognize the risks related to my volunteering and, understanding that giving a release is a condition to my participation, do hereby freely and knowingly agree to this release.

11. I agree to indemnify and hold harmless PHM from any and all costs or damages, including attorney fees, arising out of or incurred in connection with my participation in NightVision.

12. I further agree not to sue, assert or otherwise maintain any claim or cause of action against PHM arising from my participation in NightVision.

13. In the event of any dispute between me and PHM, I agree to resolve such claim by submitting the claim first to mediation and if necessary to binding arbitration pursuant to Christian conciliation.

14. I hereby give PHM permission to act on my behalf in seeking and administering medical treatment in the event that such treatment is deemed necessary or advisable for my health, safety and welfare.  I release PHM from liability in acting on my behalf in this regard and rendering any necessary medical treatment.

15. I knowingly and freely assume all risk and financial responsibility for any injury which might result from my participation in NightVision.
