Please complete the information requested below to submit your application for review. After you submit your application, you must attend an orientation and subit a self-ordered background check.

 

Attend a volunteer orientation. To help streamline the process we have now combined our new volunteer orientation with a couple of specific program trainings. If you are interested in attending a combined training for REAP or Children/Youth programs, please choose the appropriate orientation.

YOU ARE GIVING UP LEGAL RIGHTS AND ASSUMING LEGAL OBLIGATIONS.
For and in consideration of Mission Waco, Mission World, Inc. (hereafter MWMW) allowing me to participate in one or more of the activities, operations, services, special events, projects, and/or programs provided, organized, supervised by and/or affiliated, associated, or connected with MWMW in any manner, form or capacity (including those of third parties to whom MWMW makes referrals of volunteers and those which are or may be hazardous), in any location and at any time and as a condition
thereto (hereafter “program” or “programs”); and, in consideration of the benefits I will receive and/or have received therefrom, for and on behalf of myself, my estate, my heirs, my legatees, my devisees, my successors and/or assigns, I voluntarily agree as follows: I do hereby ASSUME THE RISK of any and all foreseeable and unforeseeable injury, illness, sickness, disease, death, loss or damage of any kind or type, direct or indirect, to me or my property arising in any way out of, from, or as a result of any act,omission, occurrence or event (INCLUDING THAT WHICH ARISES FROM THE NEGLIGENCE AND/OR GROSS NEGLIGENCE OF MWMW) during or after my presence and/or participation in any MWMW program; and I do hereby WAIVE any and all claims, causes of action, losses and damages of any kind or type arising therefrom and/or which have arisen in the past; and I do hereby RELEASE, ACQUIT, FULLY DISCHARGE, HOLD HARMLESS, COVENANT NOT TO SUE AS A RESULT OF, AND AGREE TO FULLY AND COMPLETELY INDEMNIFY MWMW from and against any and all claims, causes of action, losses and damages of any kind or type arising therefrom and/or which have arisen in the past (INCLUDING THAT WHICH ARISES FROM THE NEGLIGENCE AND/OR GROSS NEGLIGENCE OF MWMW), asserted or brought by anyone in any capacity, including but not limited to court costs, witness fees, attorneys fees, and all expenses of whatever type or kind associated therewith or arising therefrom and ASSIGN any and all such claims to MWMW; and, I agree that any attempt by me, or my estate, my heirs, my legatees, my devisees, my successors and/or assigns to void, nullify, avoid, cancel, modify, circumvent, revoke or set aside any of the provisions and/or operation of this document and any of the obligations imposed upon me, shall be null, void and of no force and effect. The phrase “any and all claims, causes of action, loss and damage of any kind or type” includes but is not limited to illness, sickness, disease, death, loss, personal injury (physical and/or mental), mental anguish and/or any other type of mental or physical injury, property damage and/or destruction, theft, loss of
income, loss of opportunity, loss of consortium and/or companionship of any type, any theory of recovery of damages or relief of any type, monetary compensation or property damage of any kind or  type (INCLUDING THAT WHICH ARISES FROM THE NEGLIGENCE OR GROSS NEGLIGENCE OF MWMW) asserted or brought by anyone in any capacity. The phrase MWMW is intended to include the corporate entity identified above, its predecessors, successors, parent, subsidiaries, affiliates, other related entities, associates, members, directors, officers, agents, contractors, subcontractors, employees, all persons natural and corporate, servants, volunteers, assigns, agents, contributors, legal representatives, any of those in privity with any of them and all individual and entities involved in the operation or supervision of MWMW and/or of its activities, operations, services, special events, projects and/or programs. The foregoing broad enumeration is intended to include any person, firm, corporation, entity or organization within any category enumerated whether or not specifically named and is intended to include any activity, operation, service or program of MWMW whether or not specifically named, it being my specific intent to include all such without the necessity of a detailed listing or enumeration. This document may not be revoked or voided by me. No change or modification to any of the provisions of this document may be made unless made in writing and approved and agreed to in writing by MWMW. If any portion/provision of this document is invalid, the remainder will continue in full legal force and effect. I also acknowledge that MWMW has not arranged and does not carry any insurance of any kind for my benefit or the benefit of any other person, firm or entity. I represent that I am in good health and suffer no physical or mental condition or impairment
that would or should prevent participation in any program. I also understand that this document is a contract which grants certain rights to and eliminates the liability of MWMW. I grant unto MWMW all
right, title, and interest in any and all photographic images, video, audio and electronic recordings that are made by or for MWMW, including, but not limited to the right to use and distribute such in any media or otherwise, along with any royalties, proceeds, or other benefits that are derived from such photographs or recordings. The undersigned represents that he/she is at least eighteen (18) years of age; that he/she has never been declared incompetent by a court or agency of government. The undersigned represents that no promise, inducement or agreement not expressed herein has been made to him/her to induce execution of this document and that this is the entire agreement between the parties. The undersigned has executed this document under no duress or coercion. The terms of this
agreement are contractual in nature and not a mere recital. All parties to this release shall be responsible for their own attorney's fees and expenses related in any way to any incident, except that, if a breach of
this release occurs, the non-breaching party shall be entitled to recover its reasonable attorney's fees and cost incurred from the breaching party. Notwithstanding the foregoing, no individual shall be released of any intentional tort or criminal conduct that causes bodily injury or property damage.

 

BY SIGNING BELOW, I CERTIFY THAT I HAVE READ, UNDERSTAND, ACCEPT, AND AGREE THAT I AM GIVING UP LEGAL

I. MISSION WACO VOLUNTEER CONFIDENTIALITY AGREEMENT

I understand that as a Volunteer at Mission Waco I am obligated by Mission Waco’s policy to protect the privacy of the population we serve (“participants” or “clients”) and all confidential information from unauthorized use and disclosure.  I also understand that disclosure of business and donor information of Mission Waco may harm Mission Waco’s ability to carry out its purpose.  I understand that volunteers may not be allowed to volunteer further at Mission Waco if this policy is not followed.

 

Confidential information is defined as any Participant or Client or Business information obtained through the course of your volunteer service to Mission Waco.  


• “Participant Information” or “Client Information” shall be considered any information regarding a Participant or Client obtained or learned while providing volunteer services for any program or area of work of Mission Waco.  Such information may include, but is not limited to, personal information, financial data, and social data.  When volunteering at the Mission Waco Health Clinic and/or Counseling services, additional information includes but is not limited to medical record, medical history, diagnosis, condition, or treatment. All information Volunteers learn about Participants or Clients is “protected”.
• “Business Information” shall be considered any information not publicly known regarding the business and operations of Mission Waco obtained while providing volunteer services.  This may include, but is not limited to, information concerning finances, donors, employees, and other volunteers.   
Terms of Agreement:
I agree to support Mission Waco’s policy of privacy and to advocate for and protect Participant, Client and Business privacy. I agree to not access or disclose any confidential information I learn or am exposed to as part of my volunteer duties.  I will seek the minimum amount of confidential information necessary to carry out my volunteer duties.  I agree to not post Participant or Client information on social media, and agree not to share protected Participant or Client information by email, phone, or text with anyone other than Mission Waco Staff.  I understand that photos of Participants or Clients, their families, or of staff are not permitted, without their express written permission on approved Mission Waco forms. 

 

I understand that my obligation to maintain confidentiality of information obtained from Mission Waco extends beyond the length of my volunteer service to Mission Waco.  I agree to maintain confidentiality of such information as long as it is known to me.  I understand that I may not remove any hard copy and/or electronic files of information from the premises.  I understand that in cases of safety or suspected abuse of a person, Mission Waco follows the laws of Texas and a volunteer is obligated to disclose information to a Mission Waco employee or law enforcement officer.

 

If I hear of or see a privacy breach, I will report it to the Program Director or the Mission Waco main office the same day or as soon as possible. 

 

II. ELECTRONIC MEDIA AND INTERNET USAGE

POLICY AND PROCEDURES

PURPOSE
The purpose of this policy is to provide guidelines to (full/part-time/seasonal) volunteers and employees in general, and specifically to those who:
* have access to the Mission Waco Mission World’s (MWMW) network, owned digital equipment devices, and computer hardware
* create electronic media/content that is related to MWMW business, fundraising, newsletters, and special events.

 

DEFINITIONS
1. DIGITAL EQUIPMENT - Includes but is not limited to computers, laptops, tablets, telephones, cellphones, flash drives, external storage devices, or any technology for communications, computing, printing, etc.
2. ELECTRONIC MEDIA - Includes, but is not limited to, e-mail, instant messaging, word processing documents, spreadsheets, social media, images, voice communication, and data bases. All forms of this media are considered “electronic record” and subject to the same retention laws and
regulations as any other record.
3. ELECTRONIC MEDIA RECORDINGS - photo, audio, or video
4. E-MAIL - A system for sending and receiving messages electronically over a computer network.
5. INTERNET - Worldwide communication network
6. PRIVATELY-OWNED - Not owned by the MWMW
7. SOCIAL MEDIA – Third party websites (such as Facebook, Twitter, and Instagram or other similar) where content and dialogue are created around issues/interest.


POLICY
1. OWNERSHIP
Software, data, and electronic media stored or created on MWMW-owned digital equipment/network is MWMW property. Information volunteers or employees create, transmit, or store in such equipment/network is not confidential or private. MWMW reserves the right to access such information for any purpose at any time. The Executive Director is
solely responsible to give authority to staff and/or volunteers to speak for or represent MWMW, which is typically given through job descriptions or in direct communication.
2. ELECTRONIC MEDIA SYSTEMS
a. GENERAL
* Electronic media is intended to be used for MWMW business purposes. Electronic media may not be used for any other prohibited purpose, including but not limited to the list below. Any reasonable on-going and/or temporary exceptions need to be disclosed in writing and approved by executive director.
• Private business

• Political Campaigning
• Personal online shopping that interferes with work duties
• Sites promoting exclusivity, hatred, or positions which are contrary to the MWMW’s embracing cultural diversity
• Inappropriate sites containing adult conduct, online gambling, or dating services
b. EMAIL USAGE
* MWMW email activity shall be conducted utilizing MWMW email addresses unless permission is granted otherwise.
* MWMW email system is used for MWMW business and should be used in a professional manner. Email users should read email policy and email etiquette before volunteer start date. Volunteers and employees are responsible for monitoring their incoming emails and should not accept or forward personal emails that consist of jokes, chain letters, and personal
information.
c. SOCIAL MEDIA
* MWMW volunteers and employees must maintain an online presence of high moral integrity.
Any violation of this may result in you being removed from your position.
* Excessive use of social media platform while on duty is prohibited.
3. RIGHT OF ACCESS
MWMW has the right to access any electronic media that may be located on MWMW owned equipment/network/platforms at any time without the consent of the creator/user. At any given time, the creator/user must comply with leadership requests for access to any MWMW owned
materials/content.
4. PICTURES
a. Staff and Volunteers may not post pictures of minors participating in program events on their personal social media platforms. You are allowed to share any posts from an official Mission Waco platform to your own platform. Do not use or reshare the pictures for any illegal purposes.
b. All programs will have a photo release clause for participants to sign (and legal guardians/parents signing for minors).
c. Photos/Videos/Media are allowed to be taken/shared for events/people in public spaces (for example, JFM, REAP, WCC & FTM, Clothesline, Meyer Center, etc) or events open to the public (fundraisers, community events, etc). Though other rights, restrictions and guidelines on these
pages still apply as applicable.
5. ELECTRONIC MEDIA RECORDINGS
a. Electronic media recordings made with MWMW owned digital equipment or in performance of
MWMW business are the property of MWMW and not for personal use.
b. No body worn digital recording device may ever be used while on duty
c. No electronic media recordings may be made in the following situations:
1. In areas where there is a reasonable expectation of privacy such as restrooms, locker rooms, or break rooms
2. Engaging in conversations with an employee, volunteer, or homeless person if the recording is being made without the other’s consent
3. Someone is receiving services by a healthcare professional, lawyer, or similar professionals
d. Electronic media recordings made with MWMW owned digital equipment or in performance of MWMW business may not be converted to personal use without the permission of the President
or the Executive Director.
