BACKGROUND CHECK:

I understand I will be sent an email from Backgrounds Online prior to my volunteering with Kinetic Kids. I will be notified if anything comes up that would prohibit my participation in the program, otherwise I can proceed with the volunteer process.

 

VOLUNTEER ORIENTATION:

I have attended OR will attend a volunteer orientation prior to volunteering with Kinetic Kids.

I have reviewed a copy of the Kinetic Kids Volunteer Policies and Procedures.

I understand the expectations and responsibilities of the Volunteer position, including participant and volunteer safety, and confidentiality.

I understand Kinetic Kids, Inc. staff members (including Program Instructors/Coaches and/or Program Directors) are available to assist me during my volunteer experience.

I understand participants are to remain within the activity/playing field at all times unless assisted by parent(s)/designated family member or Kinetic Kids staff.

LIABILITY RELEASE:

I assume all risks and hazards incidental to volunteering with Kinetic Kids, Inc.

I hereby waive, release, absolve, indemnify, and agree to hold harmless all Kinetic Kids staff and volunteers, as well as any facility or sponsors involved with Kinetic Kids, Inc for any claim arising out of injury to myself.

PHOTO RELEASE:

During the course of volunteering, I relieve and hereby agree to hold Kinetic Kids, Inc. free and harmless from any and all liability arising out of photographing (still or motion), and subsequent publication and/or broadcasting. I agree Kinetic Kids, Inc. may use my photograph for purposes of publication (newspaper, magazine, web pages, social media, brochures or other print media), or broadcast means of radio or television transmission.
