NOURISHKC VOLUNTEER WAVIER
Safety is of the utmost importance at NourishKC. As a volunteer, I understand that I will be working in a fast-paced kitchen. This means I may be using sharp knives, transporting hot pans, or lifting heavy boxes. Additionally, floor surfaces may become slick. Closed-toed shoes and pants are required. Once at the facility, I will also listen to instructions from NourishKC staff intended to help me stay safe. If I accidently become injured, I will notify staff immediately to receive first aid.
I am participating in the Volunteer Activities of my own free will with knowledge of the danger involved, and I agree to accept all risks of participation. I also agree to indemnify and hold harmless NourishKC and Downtown Council of Kansas City for all claims arising out of my participation in the Volunteer Activities.
In accordance with Federal civil rights law and U.S. Department of Agriculture (USDA) civil rights regulations and policies, the USDA, its Agencies, offices, and employees, and institutions participating in or administering USDA programs are prohibited from discriminating based on race, color, national origin, sex, disability, age, or reprisal or retaliation for prior civil rights activity in any program or activity conducted or funded by USDA.

VOLUNTEER NON-DISCLOSURE AGREEMENT
This Volunteer Non-Disclosure Agreement, referred to as the “Agreement”, applies to the individual whose name and signature appear below, referred to as the “Volunteer”, associated with and/or involved in the activities or affairs of NourishKC, referred to as the “Volunteer Program”.
All data, materials, knowledge, and proprietary information generated through, originating from, or having to do with the activities or affairs of NourishKC or persons associated with its activities, including contractors, is to be considered Confidential Information and the sole property of NourishKC. This includes, but is not limited to, documents, information, designs, printed matter, policies, procedures, conversations, messages (received or transmitted), resources, contacts, e- mail lists, and e-mail messages, whether internally between staff or outside the Volunteer Program, and such information is not to be disclosed to any outside party.
Any information regarding employees, volunteers, and/or guests of NourishKC, including all file information, is not be disclosed to a third party under any circumstances without the written consent of the organization. Under no circumstances may any information be printed or downloaded from any NourishKC secure platform.
Any disclosure, misuse, copying, or transmitting of any material, data, or information, whether intentional or unintentional, will subject Volunteer to disciplinary action and, in certain situations, could lead to prosecution and/or monetary damages according to any applicable laws.
The signature of the Volunteer below acknowledges his/her agreement to the terms.
PHOTO RELEASE
I understand that NourishKC occasionally takes photos or video footage of volunteers. These may be used to share the organization’s story in publications or agency online communications.
COVID-19 LIABILITY WAIVER
For my safety and the safety of others around me, agree to the following conditions for volunteering with NourishKC:
- For the protection of others, I will adhere to all safety guidelines (wearing a mask, washing hands, etc.) during the entirety of my volunteer shift with NourishKC.
- I give NourishKC permission to check my temperature upon my arrival.
- If I have a temperature of 100.1 degrees or higher, I agree to wait 2 weeks before coming back to the KCCK to volunteer.
- If I am 65 years of age or older, or if I have certain underlying health conditions, because of the known enhanced risks for contracting COVID-19, I acknowledge and accept that I am assuming any and all risk for the possibility of contracting COVID-19 while volunteering at the KCCK.
- By participating at NourishKC, to the maximum extent permitted by law, you, on behalf of yourself and your heirs, assigns, personal representatives, and next of kin, hereby release and hold harmless NourishKC and their members, employees, officers, agents, and vendors, and waives any and all COVID-19-related claims associated with my volunteer service.
