PLEASE READ BEFORE SIGNING:

 

Upon my signing of this release, I hereby volunteer to be a participant in or with Habitat for Humanity, York County (Habitat) activities. I acknowledge that participation in these events and activities involves a risk of physical injury to any individual undertaking such activities. I hereby expressly assume such risk and hereby agree to hold harmless, release, waive and discharge Habitat, its sponsors, agents, representatives or officials from any and all claims, losses, liabilities, damages or expenses I might have as a result of physical injury incurred by my participation in said activities.

 

Except as otherwise agreed to by Habitat in writing, the Volunteer does hereby release and forever discharge Habitat from any claim whatsoever which arises or may hereafter arise on account of any first aid, treatment, or services rendered in connection with the Volunteer’s work for Habitat. The Volunteer further authorizes Habitat to obtain necessary medical attention in the event of accident or illness to the Volunteer while on a Habitat work site or in the Habitat office. If there are medical conditions/allergies that volunteer believes would need to be known by anyone providing aid or assistance to the volunteer, the volunteer hereby warrants that such conditions have been listed above, or on a separate sheet of paper, and submitted with this release and waiver of liability form to the Habitat office.

 

 I also give permission to Habitat to take photos and use them in any current or future publications and communications. The Volunteer does hereby grant and convey to Habitat, any royalties, proceeds, or other benefits derived from such photographs or recordings in perpetuity.

 

I agree to abide by all rules and regulations set forth by the Volunteer Program.  This agreement shall be effective and binding upon the parties heretofore for the date indicated.

 

I have read this agreement and I fully understand its terms, conditions and meaning as the language is clear and unambiguously expresses the intent of the parties.
